Forms

Forms can be printed here; however, the ERS must have a form with an original signature(s) for processing.

All forms must be signed by the member, benefit recipient or authorized representative. In addition, some forms require a signature from a spouse, witness or Notary Public. Please be sure all necessary signatures are included.

Before submitting the form, double-check that all information and any supporting documentation is provided and correct and, if possible, make a copy for your records. Incomplete forms will be returned, causing a delay in your request.
