Important Information About the 2025  Form 1095-C

  • You do not need to submit Form 1095-C to the IRS when filing your tax return. However, we recommend keeping a copy with your tax records in case you are ever audited.
  • There is no penalty for not having health insurance coverage in 2025.
  • Form 1095-C confirms your coverage status and shows the months in 2025 that you and your covered dependents were enrolled (or not enrolled) in a healthcare plan sponsored by M-NCPPC.
  • Availability Date: The 2025 Form 1095-C will be available for benefit-eligible retirees beginning March 2, 2026.
  • How to Access the Form: You can access your Form 1095-C through Document Self Service (DSS).
